The Department of Education mandates that colleges obtain specific documentation when verifying information that students submit when applying for financial aid. Rock Valley College also requires that students read their Rights and Responsibilities and the Standards of Academic Progress policy. Depending on your individual circumstance, additional information may be necessary to process and award your financial aid.

Below is a brief description of each form for the 2009-10 academic year and its use. To view the form, simply click on the form name. Next, print the form, fill it out and submit it to the Financial Aid Office.

These forms are required from every student applying for financial aid at Rock Valley College.

1. Financial Aid Information Form - (This is a four page document. Please print and turn in all four pages). This form explains in detail the academic standards all financial aid recipients must maintain in order to receive future aid as well as the applicants rights and responsibilities.

These forms are required if your FAFSA form was selected for verification by the Department of Education or if after review by the financial aid staff more information is required to resolve conflicting information within your file.

2. Independent Verification Worksheet - You are considered an independent student if you did not need to provide your parents' tax information on the FAFSA form.

3. Dependent Verification Worksheet - You are considered a dependent student if you needed to provide your parents' tax information on your FAFSA form.

4. Illinois State Residency Form - This form provides a list of documents approved by the state to help verify Illinois residency.

Special forms

5. Curriculum Code - This form consists of a list of all programs that are considered eligible for financial aid. It is required if you do not have an eligible code recorded in the RVC Records Office.

6. Verification of Low Income and/or Support of Dependent(s) Worksheet (This is a two page spreadsheet. Please be sure to print both pages!)– You may be asked to fill this form out if income reported appears to be unusually low.

7. Statement of Registration Status - Male students between the ages of 18 and 25 are required to register with the selective service. For detailed information you can access the selective service website at www.sss.gov.

8. 2009-2010 FAFSA Signature PageYou will receive this form if you did not sign the FAFSA via electronically or you did not submit a paper signature to the Department of Education. 

9. Standards of Progress Appeal Form - Students receiving financial aid must make Satisfactory Academic Progress (SAP) toward the completion of course requirements in a certificate or degree program.  Fill out this form if you have been notified that you are no longer meeting the standards for financial aid and wish to appeal this decision. 

10. Permanent Standards of Progress Appeal Form (beyond 96 credits) - Students may receive Financial Aid until they have attempted 96 credit hours, this includes up to 43 transfer credits.
