Part 1: Complete Your Application

Students who have enrolled at Rock Valley College and would like to request accommodations due to a disability can initiate the process of getting connected to the Disability Support Services (DSS) office by completing the online application.

Completing the application will assist DSS staff in understanding more about you, any challenges you may be experiencing, and what accommodations or supports may be most helpful. The information you provide will also help the Accommodations Specialist to prepare for your initial appointment and to ensure that your needs are addressed as thoroughly as possible. Therefore, it is important that you complete the application as accurately as possible.

The application is a two-part process that consists of (1) completing information about yourself and your background and (2) uploading documentation regarding your disability.

Part 2: Upload Your Documentation 

Documentation varies depending on the nature and scope of the disability, but typically includes the following: a diagnostic statement identifying the disability; the functional impact(s) of the disability; recommendations for accommodations; and signature, credentials, and contact information of the evaluator. If you have documentation, please upload this in STEP 2 of the online application.

If you do not have documentation for your disability, please complete the Disability Verification Form.  You can contact our office to obtain a copy of this form.  If you do not have a documented disability, but need assistance, please contact us to schedule a meeting with our Accommodations Specialist. She can help with identifying options for assessing potential academic barriers, understanding how the barriers relate to your disability, and (if applicable) provide options for obtaining supporting documentation.
